Sobeys is hiring a Pharmacist (Term, Full-Time) to join its Sobeys Pharmacy at Highland in Kitchener, ON. If you are a licensed pharmacist who enjoys patient-centred care and clinical services in a community setting, this role could be an excellent opportunity.
ABOUT THE JOB
As a Term Pharmacist, you will provide professional clinical services, ensure safe and accurate dispensing and support the overall operation of the community pharmacy. You will work closely with patients, caregivers and health-care providers to help improve health outcomes.
KEY RESPONSIBILITIES
- Deliver patient-centred care with warmth, empathy and professionalism while acting in the patient’s best interest.
- Provide clinical services such as medication reviews, adherence support, health risk assessments and immunizations/vaccinations where permitted.
- Verify prescriptions for therapeutic appropriateness and technical accuracy according to regulatory and company standards.
- Counsel patients and caregivers on medication use, side effects, interactions and disease management.
- Initiate and maintain care plans, follow up with patients and complete accurate documentation.
- Collaborate with physicians, nurses and other health-care professionals to optimize therapy and support shared care.
- Supervise and delegate appropriate technical functions to Pharmacy Technicians and Assistants.
- Maintain current knowledge of pharmacy legislation, professional standards and clinical guidelines.
- Ensure compliance with all regulatory requirements, Company policies, processes and procedures (including OH&S, Loss Prevention and CQI).
- Contribute to a positive team environment focused on patient safety and high-quality service.
